Complete and Return the Intake Form
Download the SSEA Intake Form from our Client Forms page. Once completed, return by email or text prior to our consultation. Not all requested info will apply to every case, so just fill out the sections that are relevant to you. Gathering this information saves time during the call and allows us to focus on your concerns, rather than spend time going over basic details.
Sign and Return the Client Agreement
The SSEA Client Agreement details all terms of service. Review carefully and return your signed copy by email or text. You can download the agreement from our Client Forms page. If you have questions about the agreement before signing, please don't hesitate to reach out.
Pricing Information
All services are billed at a flat rate of $75 per hour. A deposit of $150 will be collected at the time the client agreement is signed, prior to the start of services. The first two hours of service will be deducted from this deposit, and all additional hours of service will be billed in quarter hour increments on a monthly basis.
